Greetings, This is a problem that libxml 2.7.4+ so unkindly introduced upon the world, but we must deal with it. Have a look at the language-snippets file in en/ (or in an active translation like japanese/french/spanish) and note all the following all over the place: xmlns="http://docbook.org/ns/docbook" That solves the problem. Basically <foo> turns to <foo xmlns="http://docbook.org/ns/docbook"> there, due to this namespace issue. There are a couple of other files that contain entities that must be updated too, namely language-defs.ent and extensions.ent come to mind (I think that's all).
