This bug has been fixed in CVS.
Snapshots of the sources are packaged every three hours; this change
will be in the next snapshot. You can grab the snapshot at
http://snaps.php.net/.
Thank you for the report, and for helping us make PHP better.
FYI Jani: -q for cgi sets headers_sent setting to 1 right a way, hence
the different result.

Verified, system() calls sapi_flush()

On FreeBSD with 5.2.9 the SAPI is apache2handler
The MacOSX 5.2.9 and 5.3.0RC1 is cli.
Do you still need me to try this on Linux? If so, I could compile
5.2.9 and 5.3.0RC1 on Ubuntu tomorrow.

I can not reproduce this with Linux. Please try without the Suhosin
patch.
