my table looks like this
CREATE TABLE
volunteer (
volunteer_id integer PRIMARY KEY,
origin_id int(10) NOT NULL,
name varchar(200) NOT NULL,
UNIQUE (name)
);
Works via command line, not through apache (with and w/o entropy)
Reproduce code:

$db = new PDO('sqlite:mealdata.sqlite');
$querystring = "INSERT INTO volunteer (origin_id , name)
VALUES
(3, 'my name')";
$query = $db->exec($querystring);
echo $query;
Expected result:
----------------
1
Actual result:
--------------
0
