This bug has been fixed in CVS. You can grab a snapshot of the
CVS version at http://snaps.php.net/. In case this was a
documentation
problem, the fix will show up soon at http://www.php.net/manual/.

Any PHP page containing a dl() call will trigger a message:
[error] PHP Fatal error: Call to undefined function:
mgd_get_midgard() in
/Users/martin/Documents/CWA/SSC/ssc_core_cvs_trunk/local/apache_1.3.26/libexec/midgard-root.php
on line 14
This is apparently known, and noted in the PHP 4.2.0 release notes.
