Sorry for the delay. Was on vacation.
Still no go. Tried with 4.2.3 200207251200 and 2.0.39. Same result as
before. Lines from httpd.conf ...
LoadModule php4_module modules/libphp4.so
DirectoryIndex index.html index.html.var index.php
AddType application/x-httpd-php .html .phtml .php3 .php
AddType application/x-httpd-php-source .phps
Also noticed in phpinfo() that there is no 'HTTP Headers Information'
section either. I can e-mail my phpinfo() info if you wish.

Yes but the behavior can be overridden with
register_globals = on in php.ini. I used the same php.ini
for both my production 1.3.2x apache binary and the 2.0.x
apache binary.
apache 1.3.2x = getenv(_ANYTHING_) = works.
apache 2.0.3x = getenv(_ANYTHING_) = no worky.
i hope this is clearer.

In PHP 4.2.0, the 'register_globals' setting default changed to
be off. See http://www.php.net/release_4_2_0.php
for more info.

part of our efforts to make PHP scripting more secure and portable.
