please tell me how can generate word file using php and mysql. this word file also include image.

This error message happens when trying to instantiate MS WORD on a system with improper permissions.

to solve it.

By well known example I assume you mean something like the
following: http://www.pastebin.ca/338618.
I have tried this example against the latest level of PHP 5.2 and it works fine. Can you please
retry with the latest 5.2 snapshot and re-open the defect with details of your actual php script if
the problem persists.

my line 13 is:
$word = new COM("Word.Application");
... the same as expected
$word->Documents->Add();
Expected result:
