Very sorry about the 'me too', but 'me too'. I hope this can get some attention,
as it would make XMLReader super useful for massive xml bodies!

XMLReader should be able to work on an open stream.

The extension is already stream-aware so it's only a matter of xmlAllocParserInputBuffer() the
xmlParserInputBuffer as in php_libxml_input_buffer_create_filename() (see ext/libxml/libxml.c) and
load that with xmlNewTextReader(), providing a nice method for that (open_stream?,
attach_to_stream?)
