From 5.6.29, 7.0.14, and 7.1.0 looks like this can now be solved with:
opcache.validate_root=1

Same issue with the following configuration:
PHP Version: 7.1.10
OS: Debian 4.9.30
Apache: 2.4.24

Just hit this nasty.
"opcache.revalidate_freq=0" does seem to be working for now (only tested briefly),
although it's not really a very good solution.
I saw mention in #72521 that a cache prefix would probably work, how could I set this up? Or does
such a feature not exist yet.
Would be nice if this could be looked into as per user pools are basically useless at the moment.

Did you try setting 'opcache.revalidate_freq=0'? Forcing opcache to revalidate the file on
every request circumvented the issue for me.
But I agree that this should be fixed in opcache (e.g. by including the inode or the fpm pool name)
