I see that my configuration lacked the recommended
opcache.file_cache_fallback=1 and
opcache.file_cache="C:\windows\temp" for Windows installations, which I have
just added. Although file_cache_fallback=1 is the default on Windows, it apparently has no effect if
file_cache is undefined.
