### Description

The following code:

```php
<?php

$all = ['test'];

foreach ($all as &$item) {
  $all += [$item];
}

var_dump($all);
```

Resulted in this output:
```
array(1) {
  [0]=>
  &string(4) "test"
}
```

But I expected this output instead:
```
endless foreach
```
