* in html, upload 3 files using below form

<input type=file name=some_duplicated_name>

* in upload.php of web-server
// below will display only 3rd entry, not 3 entries.
// if user didn't upload 3rd entry then no files uploaded.
// in apache tomcat, all 3 files uploaded correctly
