Make sure you have ENCTYPE="multipart/form-data" in the <FORM> tag, otherwise it will not work. You should have a begin and end <FORM> tag and make sure it has the ENCTYPE attribute, and the METHOD='POST' and the ACTION attribute filled in, too.
