I set them up as environment variable. I did not put them in the php
script. Do I have to set them up still with putenv() even though the are set
up in environment variables?

Are your Oracle environment variables set up ?
e.g.
putenv('ORACLE_HOME=/.../oracle');
putenv('TNS_ADMIN=/.../oracle/network/admin');
