I had to create the same thing, and it took me a while to figure out how to get the calendar to start on the right day and be the right number of days. I got it figured out for you at http://www.php.net/manual/ref.datetime.php See my post for joey.garcia@usa.net. I didn't integrate the database but that would be easy, the hard part is building the calendar. I hope this works for you.
