Just build the SQL statement with IFs...
sql = "SELECT * FROM table WHERE ";
if ($SearchByName) $sql .= "name = $Name AND ";
if ($SearchBySurname) $sql .= "surname = $Surname AND ";
if ($SearchByCompany) $sql .= "company = $Company AND ";
...
then just remove the last four characters, and perform the query...
