Bug description: Problems when ISAPI not cached
The ISAPI module has problems when it is repeatedly loaded and unloaded. If you uncheck the
'Cache ISAPI application' option in IIS4 you eventually get a 'Cannot create a file
when that file already exists.' error. From then on you always get that error.
On a different web server, the module gets as far as asking for cookies and then just exits. The
next request is fine. There seems to be no fixed number of requests between each occurence of the
problem.
