zend_fetch_resource*() change

I've converted most of the modules that use zend_fetch_resource*() to compile under the new API; I've tested only the MySQL module (which I actually changed to take advantage of the change), and modified several other modules without actually testing they compile (if they compile cleanly, they should work just as before). There are two modules which use zend_fetch_resource*() that I haven't touched - the ODBC module and the Oracle module. Authors - please update them (if you can take the time to make them use the getParametersEx() API - great, but right now they won't compile at all).
