Did you write a php script to handle the received file?
If it doesn't work anyway, there are some alternatives:
Sometime I wrote a perl script for this RFC1867 multipart messages.
I can send this to save your head ;)
There are also some Perl/CGI ressources for this upload stuff:

But you should have ActivePerl running on your NT box for it.
Btw. Webservers partly support this upload stuff, IIS with the 'Posting
Acceptor' extension. Or Pi3Web, a free webserver project I participate,
has support for upload forms from version 2.0.
