According to PHP4 ChangeLog, the following happend after RC1:

* sapi/apache/mod_php4.c:
- Fixed several problems with the PATH_TRANSLATED and PHP_SELF
under Apache (Paul Gregg & Zeev)
Your example page shows that you are still running RC1 and thus doesn't
show PATH_TRANSLATED, so you are effectively giving filectime() nothing.
Please upgrade. The problem is already fixed
