Bad news: I tried several variants - removing all of the extension
configuration, removing
everything other than the language options, and removing everything other than [PHP] - and there was
no change in behaviour.
Curious news: If I ignore the installation instructions and do not add
PHP4ISAPI.DLL to the ISAPI
Filters but do configure it to handle the .php extension under Application Mappings, phpinfo()
starts reporting that it's running using ISAPI and everything appears to work in my (admittedly
brief) testing so far. A quick wload test wasn't enough to kill the ISAPI module as it did
before,
but it's certainly possible that it would have broken under higher load settings.
