There is no problem with building the ApacheModule as a .dll on Win32.
Daniel Beulshausen and Andreas Otto are currently working on a german
article on building PHP 4 and its extensions on Win32.
Andreas Otto regularily builds PHP 4 from CVS as Win32 CGI / ApacheModule
and puts his results online at

I am working on an article about general themes with running PHP 4 on the
Win32 platform, including the build process using MS VisualC++ 6.0 ... in
English :-)
