You must specify the path in php4 to the mysql client library location.
For example (mysql installed with --prefix=/usr/local/mysql):
./configure \
--with-apxs=/usr/local/apache/bin/apxs \
--enable-versioning \
--with-mysql=/usr/local/mysql
--enable-track-vars
Re-open if problem persists.
