Build | Install Tracker for PHP

I have posted a rough form for a build and install tracker at http://qa.php.net/build_tracker/ Please let me know if any additional fields need to be added, or if any of the existing fields should be modified. I am planning to parse the list of installed modules out of the configure line. Please send me your comments, suggestions and criticisms. Also, in the logo frippery vein, note the little logo in the corner - comments suggestions? - zak
