The following SAPI modules define it:
Apache
CGI
ISAPI
NSAPI
Java Servlet
We can probably move the definition one level up so that it
is available everywhere. I'll look into that.

It is better than your proposal. Your proposal assumes that
your include file will be included by the main app and not by
other helper files (like prepend.php).
