Bug description: each() returns empty [0] / [key] field
after adding a new element to an array using "array[]=..." the each() function returns an
empty [0] / [key] field for this new element. the [1] field contains the correct value.
setup: standard 4.0.3pl1 distribution for win32 (php.ini = php.ini-dist)
