Bug-fixes and new functionality is intermixed in our development process. I'd propose copying the Linux/Apache/MySQL approach, where the first digit is bumped in case of a serious infrastructure change/rewrite; The middle digit is bumped for things that strongly affect the way you use PHP (e.g., turning register_globals off by default, or something equally far-reaching); The 3rd digit is bumped for everything else.
