ldap_add, ldap_modify cause kill of four apache processes in the case of multiple values of an
attribute, when they are not indexed using integers starting with 0.

entry["attribute0"][4] = value0
entry["attribute0"][5] = value1

There should be an error message or warning preventing this kind of entry indexing.

php 4.0.6
OpenLDAP 2.0.11
Apache 1.3.17
Solaris 7
