problem with uploading using type=flag tag

Hi,
I am uploading a file using <type = file> tag.
It makes a files in /var/tmp/ corresponding to that file by itself.
Then i copy the file to a certain location....but for some file it gives the
following error:
Warning: Unable to open 'none' for reading: No such file or directory in
/z/mercury/www/data/content/admin/do_upload.php on line 40
I dont know if its the 'size' or 'timeout' which doent let the copy occur.
Can anyone tell me how can i overcome this error.
