What about other hooks like what mod_perl has:
PerlChildInitHandler
PerlPostReadRequestHandler
PerlInitHandler
PerlTransHandler
PerlHeaderParserHandler
PerlAccessHandler
PerlAuthenHandler
PerlAuthzHandler
PerlTypeHandler
PerlFixupHandler
PerlHandler
PerlLogHandler
PerlCleanupHandler
PerlChildExitHandler
I don't think we need all of them, but AccessHandler and AuthzHandler would be nice to write a Authentication handler in PHP. LogHandler and CleanupHandler would be nice so things can be done after the client connection is closed.
I know, baby steps, but just thinking big picture. :-) Great job!
