We do the following in our Apache virtual hosts:
<VirtualHost *>
ServerName www.example.com
DocumentRoot /var/www/www.example.com/
SetEnv city apeldoorn
SetEnv region_id 97
SetEnv module_name index
</VirtualHost>
You can then access these from PHP:
<?php
echo $_SERVER['city'];
?>
