Ok, here is a plan:
1. Use fgetc in stream_gets for stdio stream ops
2. Add file descriptor-based stream ops
3. Redirect _php_stream_fopen_from_file/pipe to new stream
ops, if possible
The first point is trivial to implement. The rest ensures
that no fread/read calls are mixed which could lead to some
pretty obscure bugs.

I'm not sure what you are referring to -- do these libraries
make use of "FILE *" directly?
