Firstly you 'must' have the gd library include files (as well as the library), selecting --with-gd=no is not allowed. Secondly, having the packages installed is not enough. One has to have the source code (or at least the header files) installed as well. The install for the postgres database is _not_ the one given by setup.. that is for the executable and data and does not include the header files. These things are not mentioned in the documentation and thus lead to problems such as I have encountered (despite having a fully working postgres ..)
