The shared-manual.inc file is part of phpweb, see
http://wiki.php.net/doc/phd/view#viewing_the_phpfolder

When I compile the php documentation with:
phd -d .manual.xml
the phpweb theme create a directory "php" for his files. In all the php
files there is an include_once:
include_once $_SERVER['DOCUMENT_ROOT'] . '/include/shared-manual.inc';
but the directory "include" and his files are not generated in the php
directory by phd.
