Bug description:Errata in draft formal PHP spec

While reading the draft formal spec for PHP, via the current version of
Firefox on a Mac, I recorded many instances where the syntax equations
or text descriptions were in error. This errata capture was done in a
flat text editor, so apologies for the lack of italics and
differentiated fonts. The resulting errata log is attached.
