We update the extension-membership of the respective extensions,
and fix the installation instructions where necessary.

Description:
------------
---
From manual page: http://www.php.net/extensions.membership
---
The Core Extensions subsection of a document contains following note:
"These are not actual extensions. They are part of the PHP core and cannot be left out of a PHP
binary with compilation options."
However, at least one of mentioned Core extensions, namely Sessions, does not fit into this
description.
---
From manual page: http://www.php.net/session.installation
---
"Session support is enabled in PHP by default. If you would not like to build your PHP with
session support, you should specify the --disable-session option to configure."
Moreover, on some systems (namely FreeBSD) sessions are actually supplied as a separate package that
is not necessary to be installed in order to use PHP.
Therefore, a conclusion can be done that either Sessions is not a Core extension or the definition
of Core extension is not accurate.
