bug report already describes this very problem. Even if you feel
that your issue is somewhat different, the resolution is likely
to be the same.

Even with newer verison chm PHP documentation.
The writing PHP Extension chapter still missing ( two chapters in
total)
I have decompressed the source file to see if it is included.
But it does't exist, but, however, it does exist on multiple-page html
source.
As it is exist on very older version.
It is started with zend*......html
