Bug description: JSON functions are not listed in the keyword index in chm help file
Description:

In the windows chm help file, you can use Keyword Index tab to look for
the function. I thought all of the function names should be listed in it,
but neither of 'json_encode' or 'json_decode' are not found in the index.
These function reference pages are found at "LXXIII. JSON Functions" of
"Part VI. Function Reference".
The problem reproduced in both of English and Japanese chm file.
Reproduce code:
---------------
1. Download HTML Help file (chm) from
http://www.php.net/download-docs.php
2. Open the downloaded windows help file
3. Move to the keyword index tab
4. Type 'j' to look for 'json_encode' and 'json_decode'.
Expected result:
----------------
'json_decode' and 'json_encode' should be listed between 'jpeg2wbmp'
and
'JulianToJD'
Actual result:
--------------
The next entry of 'jpeg2wbmp' was 'JulianToJD'.
