Maybe, you are using too old XSL stylesheets.

Hardly to say. Norm is currently on vacation, and it depends on a number
of new things added to stylesheets, but maybe before the end of
February.
If you don't want to wait, you can grab latest release, and just replace
file html/autotoc.xsl with version from CVS and add following line to
html/params.xsl
<xsl:param name="toc.max.depth" select="8"/>
