Bug description: extension_dir is wrong for Windows

The variable extension_dir is set to "./", it must be "./extensions" for
Windows users. Please state this in the php.ini file, and in the manual...
and make it default when installing the Windows exe??
When set to ./ and you enable gd2 (probably anything), and try to load a
php page, it justs loads forever.
