I just ran through the steps. There were only two minor issues:
1. "php doc-base/configure.php" appears twice. The first time is under the "Validate the PHP Documentation XML" heading. The second time is under the "Use PhD to build the documentation" heading.

2. I got two warnings when building the documentation in php format:
$ php path/to/phd/render.php --docbook /tmp/svn/doc-en/doc-base/.manual.xml --package PHP --format php --output mydocs

I also have these errors. I reported them while ago, so it seems to be known issue.

Otherwise it seems to work fine. There were no warnings with xhtml format.
