Use MySQL 3.23.X and after 'submit' the form try
mysql_query("insert into my_table set my_db_field =
load_file('$my_form_field');");
$my_form_field is the name of form field
$my_form_field_type is the file mime type you're uploading
$my_form_field_name is the name of file you're uploading
