When sendmail is dead, or during nuclear attack. :-)
Or if you misconfigure your php.ini file badly enough to not even get to
sendmail.
It's pretty rare.
If you want to validate emails, try one of the umpteen email validation
solutions. Manuel Lemos' is probably the most exhaustive, but *nothing*
will guarantee that your email got there.
