creating a .doc file in php

I am creating a 'vacation' scheduling webpage for my company. Currently, when we want a day off, we fill out a document in word and send it to our supervisor. The page I am working on will have a calendar that displays who is off when. I would like for the employees to click on a date, enter their information and then when the info is saved to the db, I would like to have the .doc automatically created with the info and sent to their supervisor. Is this possible?? Thanks, Eddie
