Check your register_globals settings! Either switch it on (bad) or start
your
scripts with $PHP_SELF = $_SERVER['PHP_SELF'] or replace all $PHP_SELF with
$_SERVER['PHP_SELF'].
If you don't know what register_globals is check the Manual!
