I can't see where the problem may be, but you may want to try this PHP form
generation and validation precisely as you need and probably much more. It
generates the necessary Javascript to make it validate forms on the client
side and you can call the class functions in PHP to validate it on the
server side.
If your problem also shows in the Javascript code generated by the class,
just mail me back with your test script, browser and OS version that you
used.
