Sending Results of a form to more than one place

Hi, I have a form I've made which, so far, lets you browse for a file on your computer, then when you hit submit the file is uploaded into a directory on my server. How can I make it so I add more text fields to the form, then when you hit submit the file is uploaded into the directory and the stuff in the text fields is sent to a mySQL database, which then appears as text on a normal page? Thanks
