PHP creates all the html on the second page.
There is nothing flash about the second form
<FORM>
<select name='curDel'><option value='1'>Comma
<option value='2'>Space
<option value='3'>Tab
<option value='4'>Other</select>
<input type='text' name='curOther' size='5'>
<select name='desDel'><option value='1'>Comma
<option value='2'>Space
<option value='3'>Tab
<option value='4'>Other</select>
<input type='text' name='desOther' size='5'>
<select name='misField'><option value='1'>Comma
<option value='2'>Space
<option value='3'>Tab
<option value='4'>Other</select>
<input type='text' name='misOther' size='5'>
<input type='submit' name='submit' value='process'>
</FORM>
This is the kind of form it will create
