Check with your post office. They may have some guidelines for addresses
in international mail that will help. However, if you're thinking
international, Japan doesn't use a street name in addresses, although
the Japanese post office's English literature talks about street
addresses and street numbers as if they do.
Don't try to be too rigid about address forms. Three lines, for instance,
is not enough. Numbers may came both before and after the smallest
non-numeric unit, whatever that is. If you insist on drop-downs for
things like "Rd.", leave an empty or "NONE" entry.
Four simple blank lines might not even be such a bad idea.
