You should anyway get the issue with $_SERVER['PHP_SELF'] sorted out ;-)
<form action="<?php echo $_SERVER['PHP_SELF']; ?>">
always works for me. Must be some type on there, _or_ you have some nasty
code before that clears the $_SERVER array, or the $GLOBALS array...
Hint: just before your form tag, insert this code:
<?php
echo '<pre>'; print_r($_SERVER); echo '</pre>';
?>
and see what it looks like.
