Just a guess, but... you always have a file in that directory. It looks
somehting like this:
<?PHP
include "header.php";
?>
...FILE_NOT_FOUND....
<?PHP
include "footer.php";
?>
Try it without the:
> php_value auto_prepend_file header.php
> php_value auto_append_file footer.php
